MCK -- Fiscal Q1 2027 earnings preview
The setup is constructive, but the hurdle is more nuanced than a simple revenue beat. The Street expects $103.88 billion of revenue and $9.56 of adjusted EPS, equal to 6.2% and 15.7% year-over-year growth, respectively. That combination already assumes McKesson converts a mid-single-digit top-line result into mid-teens EPS growth. The clean positive outcome is therefore an EPS beat supported by segment operating profit, stable prescription utilization, and continued specialty and oncology momentum -- not merely higher pass-through drug revenue.
Our base case is $103.6 billion of revenue and $9.60 of adjusted EPS. The revenue estimate applies the midpoint of management's segment growth ranges to the prior-year quarter; it is a transparent run-rate estimate, not a substitute for the unavailable internal analyst model. We expect management to reaffirm the $43.80-$44.60 FY2027 adjusted EPS range. A modest raise would be a positive surprise this early in the year, while a reaffirmation with stronger operating profit language would still be sufficient.
The most important debate is whether the mix shift is working. North American Pharmaceutical revenue can be noisy because of GLP-1 volumes, branded drug pricing, Rite Aid comparisons, and customer onboarding. Oncology & Multispecialty and Prescription Technology Solutions carry better fundamental signal. Sustained double-digit oncology operating-profit growth, continued demand for access solutions, and progress on Medical-Surgical separation would support the long-term 13%-16% EPS algorithm.

Management said the FY2027 EPS cadence should be broadly similar to fiscal 2026 between the first and second halves. It also noted that the guide represents 14%-16% underlying growth after excluding the prior-year Norway contribution and a $51 million investment gain. That makes Q1's expected 15.7% EPS growth consistent with the underlying algorithm, even though it sits above the 12%-14% reported full-year guide.

The more differentiated call is on operating profit: Oncology and RxTS should remain the strongest contributors, while North American Pharmaceutical should demonstrate that branded-price declines and GLP-1 variability do not impair economics.

Revenue growth decelerated through the year as acquisition timing, customer onboarding, branded-price changes, and Rite Aid distorted comparisons. The better signal was operating profit: it rose 13% in Q4 while revenue rose 6%. Q1 needs to preserve that operating leverage.

That divergence supports management's argument that revenue is an imperfect economic signal for a pharmaceutical distributor.

Over five years, adjusted EPS compounded faster than revenue as operating leverage and share repurchases augmented the scaled distribution model. - North American Pharmaceutical economics. Q4 GLP-1 distribution revenue rose 22% year over year but fell 4% sequentially, while branded price declines reduced segment revenue growth by about three points. Management said neither affected operating profit. Q1 should confirm that claim.
- Oncology organic durability. The segment grew Q4 revenue 35%, helped by PRISM Vision and Core Ventures. As those deals anniversary, organic growth near management's cited 13% rate becomes the cleaner test.
- RxTS profit versus revenue. Approximately 55% of segment revenue comes from lumpy 3PL activity. Access and affordability programs -- including GLP-1 support -- should keep operating-profit growth well above revenue growth.
- Medical-Surgical separation. Apollo's $1.25 billion investment closed June 1 for an approximately 13% interest, valuing the business at roughly $13 billion enterprise value. Watch the IPO timetable, stranded costs, incremental debt, and noncontrolling-interest accounting.
- Cash conversion. FY2027 free-cash-flow guidance is below the $5.41 billion FY2026 result. Management attributes much of the difference to unusually favorable prior-year working-capital timing. Q1 commentary should support that explanation.

- CFO transition: Britt Vitalone retired after the fiscal Q4 call. The team should listen for continuity in guidance discipline and capital allocation.
- Revenue-quality optics: High drug-price pass-through can move revenue without changing profit; a revenue beat with weak segment profit is low quality.
- Policy and pricing: Inflation Reduction Act price changes, biosimilar adoption, and branded manufacturer price declines can distort reported growth.
